Job Objective:
A brief overview of the position.
A professional Registered Nurse whose clinical expertise and training guides patients, families, and caregivers to make informed decisions, collaborating with a multidisciplinary team to allow for timely screenings, diagnosis, treatment, and increased supportive care across the care continuum.
In essence, nurse navigators guide patients through the healthcare system and help them to overcome barriers to optimal care.
The Nurse Navigator demonstrates leadership/management skills and a commitment to professional accountability and growth.
This Position may require working at alternate sites.
This position will also require care/management of other patient populations (pediatrics to geriatrics) as seen in clinic.

Experience Required:
2 - 5 years in clinical setting.
Knowledge of current nursing, medical and health trends and practices in rendering of patient care.

Collect comprehensive data pertinent to the patient's health or situation.
Diagnosis:

Planning:
Develop a plan that prescribes strategies and alternatives to attain expected outcomes.
Implementation:
Implement the identified plan.
Evaluation:
Evaluate progress toward attainment of outcomes.
Quality of Practice:
Systematically enhance the quality and effectiveness of nursing practice.
Participates in Patient Centered team meetings and quality improvement initiatives through the Nursing Practice Council.
Education:
Attain knowledge and competency that reflects current nursing practice.
Facilitates health and disease patient education, including leading group office visits.
Supports patient self-management of disease and behavior modification interventions.
Professional Practice Evaluation:
Evaluate personal nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations.
Promotes clear communication among a care team and treating clinicians by ensuring awareness regarding patient care plans.
Collegiality:
Interact with and contributes to the professional development of peers and colleagues.
Participates on a team for data collection, health care outcome reporting, clinical audits, and programmatic evaluation.
Collaboration:
Collaborate with patient, family, and others in the conduct of nursing practice.

Utilizes the ethic decision making model to drive issues and advocate for patient wishes.
Research:
Integrate research findings into practice.
Monitor the progress of patient through the service plan and re-evaluate the effectiveness and appropriateness of the plan based on the patient's condition and needs.

Consider factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing services.
Provide patients linkages to services to promote a pathway to care and positive outcomes.

Provide leadership in the professional practice setting and the profession.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
