The Role:
As a Hospice Registered Nurse Case Manager, you will play a pivotal role in providing holistic care to our patients and their families.
Your responsibilities will include:
Conducting initial assessments and developing personalized care plans.
Administering medications and providing symptom management.
Collaborating with a multidisciplinary team of healthcare professionals.
Offering emotional support and education to patients and families.
Ensuring that patients receive the highest quality of care during their end-of-life journey.
Qualifications:
Current RN license in California.
Hospice and palliative care experience preferred but not required.
Strong communication and interpersonal skills.
Compassionate and empathetic nature.
Ability to work collaboratively in a team environment.
